About the Position Job Title: Registered Nurse Profession: Nursing

Experience: A minimum of one (1) year experience as an RN, either
in acute care hospital, oncology, long-term care, hospice, or home
health. License: Current license to practice nursing in the state
of employment Certifications: BLS (American Heart Association)
Must-Have: Knowledge of terminally ill patients and their families,
knowledge of hospice, excellent patient assessment skills, strong
oral and written communication skills, ability to operate nursing
assessment tools and technology, and reliable transportation for
work. Description: The Registered Nurse (RN) will provide direct
patient care services to hospice patients and supportive care to
the patient's family. The RN will manage an interdisciplinary group
consisting of Licensed Practical/Vocational Nurses and support
staff to enhance the quality of hospice care. The RN is responsible
for case managing all assigned patient clinical records. Assures
the highest quality of care is provided and documented in
accordance with the Plan of Care. Performs the initial and ongoing
assessment of the patient. Helps develop and implement the Hospice
Plan of Care as prescribed by the physician and in conjunction with
other members of the Interdisciplinary Group. Works closely with
the Interdisciplinary Group to provide physical, psychosocial,
educational, emotional, and spiritual services, including
bereavement support to patients and families. Reports any change in
the patient’s condition to both the Attending Physician and Hospice
Physician. Maintains an accurate and updated medical record,
including all Interdisciplinary notes, Plans of Care, and
assessments. Practices infection control measures and standard
precautions. Monitors scheduling of all staff. Participates in the
Quality Assessment and Performance Improvement Program. May be
requested to participate in the on-call schedule. Consistently
promotes core values. Completes required compliance training
annually. Performs other activities as assigned. Attends staff
meetings. Maintains a high level of knowledge regarding pain
management and symptoms in the dying process. Makes routine
supervisory visits of all team members at least once a month.
Conducts ongoing reviews and follow-ups on assigned patient
clinical records to ensure quality services and compliance with
policies and regulatory requirements. Maintains close contact with
the patient’s family to provide information, support, and
continuity of care. Acts as the primary liaison between the
physician, patient/family, and the hospice team to remain informed
on all clinical issues, staffing, and scheduling concerns.
Responsible for educating the patient, family, or other caregivers
about needs identified in the Plan of Care, including medication
administration, dressing changes, and oxygen administration.
